Who Should Be Tested?
The National Osteoporosis Foundation recommends that the following individuals consider osteoporosis screening:
- Women aged 65 and older.
- Men aged 70 and older.
- Postmenopausal women and men aged 50 to 69 with risk factors such as family history, low body weight, or use of certain medications.
The American College of Physicians suggests that all women aged 65 and older and those over 50 with a history of fractures receive a bone density test.
Diagnostic Tests for Osteoporosis
Osteoporosis is primarily diagnosed using bone mineral density (BMD) tests. The most common and widely used test is the dual-energy X-ray absorptiometry (DEXA or DXA) scan.
DXA Scan
A DXA scan measures the density of bone in the hip and spine, providing an indication of bone strength. It is a quick, non-invasive procedure that typically takes about 10 to 30 minutes. The results are presented as a T-score:
- A T-score of -1.0 or above is considered normal.
- A T-score between -1.0 and -2.5 indicates low bone density (osteopenia).
- A T-score of -2.5 or lower suggests osteoporosis.
Other Diagnostic Tools
Besides the DXA scan, other tests and methods may help diagnose or assess the risk of osteoporosis:
- Quantitative Ultrasound (QUS): This test is often used for screening purposes and measures the heel's bone density. While not as comprehensive as the DXA scan, it offers a quick and radiation-free alternative.
- Peripheral DXA (pDXA): This involves a less powerful version of the full-body DXA scan and is typically used to measure bone density in the wrist or finger.
Interpretation of Results
Interpreting the results of a bone density test should always be done by a healthcare professional who can consider your overall health, family history, and any other risk factors for osteoporosis.
A low bone density result does not necessarily mean you will experience fractures, but it indicates the need for lifestyle changes or potential treatment to manage and reduce the risk of complications.
