Autophagy: What It Is, How It Works, and Whether Fasting Really Helps “Clean” Cells

Autophagy is a natural cellular process through which the body breaks down and recycles damaged or unnecessary cellular components. In simple terms, it works like a cell’s own recycling system: old proteins, damaged cell structures and certain organelles are broken down into components that can be reused. This process plays an important role in maintaining cellular health and balance.

How does autophagy work?

Autophagy is controlled by several complex signaling pathways, including AMPK and mTOR. When nutrients and energy are readily available, mTOR promotes cell growth and the production of new cellular components. When energy availability decreases, the body activates adaptive mechanisms, including increased autophagic activity.

This is why researchers are particularly interested in fasting, calorie restriction and physical exercise. However, the popular claim that autophagy begins after exactly 16, 18 or 24 hours without food is not scientifically established in humans. Measuring autophagy directly in living people is difficult, and the process can vary between tissues.

Does fasting increase autophagy?

Animal studies provide strong evidence that nutrient deprivation can stimulate autophagy. Evidence in humans is more limited but is growing.

A 2025 study involving 121 people with obesity examined the effects of six months of calorie restriction and time-restricted eating. Researchers found significant changes in markers associated with autophagic activity in the time-restricted eating group compared with standard eating patterns. However, this does not prove that a specific fasting schedule can “rejuvenate” cells or extend lifespan.

Another 2025 study involving 50 healthy adults examined changes in autophagy-related pathways during 30 days of Ramadan fasting. Researchers observed changes in several markers associated with autophagy, but the findings do not establish a universal fasting duration required to achieve a maximum effect.

Can exercise stimulate autophagy?

Physical activity may also influence autophagy. A 2025 systematic review and meta-analysis of 26 human studies found that the response can vary depending on the type and duration of exercise and the tissue being studied.

This means that exercise should not be viewed simply as a way to “maximize” autophagy. Regular physical activity provides many well-established health benefits independently of its potential effects on cellular recycling.

Do you need to fast specifically for autophagy?

At present, there is not enough evidence to recommend prolonged fasting specifically to activate autophagy in healthy people. Research into the relationship between autophagy, fasting, aging and disease prevention is still developing.

If someone chooses time-restricted eating, the overall quality of the diet remains important. The eating window should still provide adequate protein, fiber, vitamins, minerals and total energy.

Fasting may also be inappropriate for some people, including those who are pregnant, people with certain chronic diseases, individuals taking medications that affect blood glucose, and people with a current or previous eating disorder. Prolonged fasting should not be undertaken without appropriate medical advice.

The bottom line

Autophagy is a real and important biological process that helps cells remove and recycle damaged components. Fasting and physical activity can influence autophagy, but there is currently no strong evidence that autophagy is automatically “switched on” after exactly 16 hours of fasting.

Rather than trying to maximize one cellular mechanism, a more evidence-based approach is to focus on established foundations of health: a balanced diet, regular physical activity, sufficient sleep, healthy body weight and appropriate preventive medical care.
